Jay passed away on Thursday, August 30, 2018 surrounded by his wife Renee and son Matthew.
Jay's family and friends will gather at Sax-Tiedemann Funeral Home & Crematorium, 9568 Belmont Ave., Franklin Park, on Saturday, September 15, 2018 from 2:00 pm until 6:00 pm to celebrate his life.
The times of laughter, tears, and the times that drove us crazy will all be a part of this special celebration of Jay's life.
